What is rehabilitation care?

Rehabilitation care, sometimes called convalescent care, is short to medium-term support for people recovering from illness, surgery, a fall, a stroke or another health event that has temporarily affected their independence or physical ability.

The goal is not just recovery. It is to help someone regain as much confidence, strength and independence as possible, so they are ready to return home when the time is right. For some people that happens quickly. For others, it takes longer. Either way, the focus is always on progress.

Our team supports people through exactly this kind of recovery. We provide nursing and residential care alongside activities and therapies designed to help residents rebuild strength and routine, with personalised support from a team that genuinely wants to see them do well.

Who is rehabilitation care for?

Rehabilitation care at Swan House may be right for your loved one if they are:

  • Recovering from a stroke, fall, operation or serious illness
  • Being discharged from hospital but not yet ready to manage fully at home
  • Rebuilding physical strength, confidence or mobility after a health event
  • In need of nursing or residential support during a period of recovery, with a view to returning home

Some people come to Swan House knowing they will stay for a matter of weeks. Others find that their needs change during their stay and choose to remain with us longer term. We are flexible, and we will always be honest with you about what we think is best.

Recovery is personal, and no two people follow exactly the same path. That is why we do not take a one-size-fits-all approach at Swan House. We take time to understand each person’s needs, goals and home situation, so we can put the right support in place from the start.

Our team includes registered nurses and experienced care staff who support people through recovery with warmth, skill and encouragement. Alongside personalised care, residents benefit from comfortable single rooms, good food and a welcoming environment that helps them feel stronger and more like themselves again.

The focus is always on building confidence as well as strength. Whether someone is regaining mobility, settling back into daily routines or working towards returning home, we are here to support steady progress in a way that feels reassuring and achievable.

What is rehabilitation care in a care home?

Rehabilitation care - also called convalescent care - is short to medium-term support for people recovering from illness, surgery, a fall or a stroke. The aim is to help someone regain as much strength, confidence and independence as possible, so they can return home when they are ready.

Who is rehabilitation care suitable for?

It is suitable for adults of any age who need a period of supported recovery - particularly people being discharged from hospital who are not yet ready to manage alone at home, or those recovering from a stroke, operation or serious illness who need nursing or residential support alongside their recuperation.

How long does a rehabilitation stay last?

It varies depending on the individual. Some people recover and return home within a few weeks. Others need a longer period of support. We are flexible, and we will always be open with you about progress and what we think is the right next step - including if someone's needs change and a longer stay makes more sense.

What is the difference between rehabilitation care and respite care?

Respite care is primarily short-term care that gives family carers a break, and the person receiving care may not have a specific recovery goal. Rehabilitation care has a clearer focus on recovery and regaining independence, usually following a specific health event. In practice, the two can overlap, and our team at Swan House is experienced in supporting both.
